Assessments & Care Plans

A thorough physiotherapy assessment of each resident’s physical function.
In consultation with the resident and nursing staff, strategies are formulated that are specific to their individual needs, such as developing a structured and individualised exercise program.
Conducting regular reviews to evaluate the effectiveness of the physiotherapy and mobility care plans for each individual resident.

Comprehensive Falls prevention System

Falls are reviewed and incidents analysed to determine causes.
Incident reports are complied together to identify trends.
Recommend falls prevention strategies and wherever possible, we will assist with implementing them to reduce the risk of falls and associated injuries.
Linking residents into other physiotherapy and allied health services as required.
